Outstanding Service Award

An outstanding member of the community will be honored and specially recognized during the 5th Annual Celebration of Life Benefit on April 2, 2011 sponsored by the Katie Haumesser Foundation.  The Foundation is seeking nominees who have provided outstanding service or who have made a significant contribution to the community in areas that serve children.  The successful candidate will be honored at the event with a special presentation and award.  Nominees must exemplify the love of caring for or about children; demonstrate exceptional commitment, service, or leadership; be of strong moral character; and serve as a role model for others in the community and “lead by example.”  Information about the upcoming event can be found on the event tab or by calling 513-602-3763. Applications will be accepted through February 18, 2011.  

Since its inception, the Katie Haumesser Foundation has raised and donated over $50,000 in Katie’s memory.  Its mission is to support the causes that Katie believed in:  raising awareness of issues that affect children with special needs, providing financial support for programs serving children with special needs, and providing academic scholarships to students entering the field of Speech Language Pathology.

Applicant Eligibility


·         There are no age restrictions and nominees may include youth, young adults, seniors, families, and groups.


·         Candidates may be self-nominated or nominated by their group, club, or project, or by an individual.


·         The nominee must have a record of voluntary or professional commitment to children. 


·         The nominee must be of exemplary moral character.


·         The nominee must provide service to children within the Greater Cincinnati area.


·         Applications for candidates under the age of 21 must be endorsed by two adult members of the nominating group, club, or service.


·         All nominations will be assessed by the Board of Directors of the Katie Haumesser Foundation.


·         All inquiries should be made to Amy Haumesser Zeisler, Chairman. 


·         Deadline for nomination is February 18, 2011.

It is important to remember that the Board of Directors will make its decisions based on the information received.  Please ensure all information is factual, verifiable, and provides a comprehensive overview of the nominee’s service to children.

1.      Name, address, and contact information of the nominating group, club, project, or individual


2.      Name, address, and contact information of two adults who endorse the nomination of a candidate under the age of 21.

2.      What is the nominee’s length of involvement and the amount of time devoted on a weekly or monthly basis to helping children?


3.      Describe the nominee’s activities, contributions, or involvement in helping children.


4.      What particular needs of children are being served by the nominee?


5.      Describe the nominee’s participation in training / skill development.


6.      Describe the nominee’s impact on the children he or she serves.


7.      What is the primary reason(s) your nominee should receive the 2010 Katie Haumesser Foundation Outstanding Service Award?
